About Medical and Healthcare Invoice Templates
Besides the templates above, this page has two more free files. The Excel invoice bills by hours and rate and calculates the subtotal, tax and total. The Word invoice is a simple layout with a description and an amount on each line. They are general service invoices, not separate dental, therapy or veterinary templates, so rename the lines to fit your practice.
What to include on a healthcare invoice:
- the patient's name (for a vet, the owner's and the animal's name) and the date of each visit;
- each service or treatment on its own line with its fee; practices that bill insurers often add procedure and diagnosis codes;
- products or medication supplied, on separate lines;
- payments already received from the patient or an insurer, and the balance due;
- your payment terms and how to pay.
An invoice is not an insurance claim. Claims usually go on the insurer's own forms (in the US, for example, the CMS-1500 or its electronic version), and some patients ask for an itemized statement, often called a superbill, so they can claim reimbursement themselves. Keep medical details on an invoice to what the patient needs, and follow the privacy rules that apply to you, such as HIPAA in the US. Our medical billing and coding guide answers common questions.
